The Cuyahoga County Board of Health is looking into a cluster of illness among individuals who may have spent time at Surrarrer Elementary School in Strongsville.
We asking all who were at the school, or at the school tobogganing event on January 15, 16, and/or 17, 2024, to complete a questionnaire with us.
We wnat to speak with both those that were ill AND those that were not.
The information that you provide is confidential. The questionnaire will take about 10-15 minutes.
