2024 Data to Care NIC List Data Brief

The D2C NIC List Analysis 2024 summarizes how the Cuyahoga County Board of Health identifies and supports individuals with HIV who may not be engaged in medical care. Using HIV surveillance data, the Data to Care (D2C) program generates a Not‑in‑Care (NIC) list each year to locate people who have not had recent HIV‑related labs, medical visits, or ART prescriptions.

In 2024, 204 clients were identified on the NIC list— a 10% decrease from 2023. Most clients were male, lived in Cuyahoga County, and represented diverse age and racial groups. Outreach efforts confirmed that 66 individuals (32%) were already in care, 6 individuals (3%) were confirmed not in care, and 8 clients (4%) were linked to support services.

The report highlights continued improvements in identifying out‑of‑care individuals and emphasizes ongoing collaboration between surveillance teams, DIS staff, and Ryan White Part A providers to strengthen linkage and retention in care.
