H1N1 Vaccine Provider InformationThe Cuyahoga County Board of Health recognizes that healthcare providers are integral in the delivery of the H1N1 vaccine in our community. CCBH's role in the vaccine delivery in Cuyahoga County has included:

H1N1 Vaccine DistributionAllocation and distribution of the vaccine is being handled through the Ohio Department of Health. The local health department usually has access to the ODH weekly allotment list on Mondays for the current week. Other than the weekly allotment list, the local health department has no prior knowledge of when individual providers will receive the vaccine from the distributor. To inquire if your practice will receive a shipment for that current week contact Romona Redding at 216-201-2001 ext 1317 or rredding@ccbh.net. Please send an email or leave a message with the provider and practice name, phone number and/or email address. Vaccine ReallocationVaccine RequestsThe Cuyahoga County Board of Health may be able to reallocate vaccine to VFC providers and/or pre-registered providers who have been inspected by CCBH to receive the vaccine. Reallocation will be based on vaccine availability and the type and amount that you request. Completion of this form does not ensure your practice will receive vaccine from the local health department. Providers will need to arrange for pick up from our Parma location. You may still receive vaccine directly from the distributor via the Ohio Department of Health regardless of vaccine received from the local health department. Click here for the reallocation request form. Fax the form to 216-676-1319. Vaccine ExcessNow that vaccination is open to the public, CCBH is requesting that you keep excess vaccine. If you are a provider that has received a type of vaccine you cannot utilize (ex. nursing home receiving pediatric doses), CCBH can pick up the vaccine for redistribution. Click here for the vaccine pick up form. Fax the form to 216-676-1319. New Allergist RegistrationThe Ohio Department of Health is extending the opportunity for
allergists to register as providers of the H1N1 influenza vaccine. According to the Centers
for Disease Control and Prevention, a
regimen has been developed for administering influenza vaccine to
asthmatic children with severe disease and egg hypersensitivity (J Pediatr 1985;106:931-3.).
